How to get a map of Colorado Medical Marijuana To obtain a Colorado Medical Marijuana card, you must have a qualifying condition. Under Colorado law eligibility requirements include but are not limited to:
AIDS / HIV
Arthritis
Asthma
Cachexy

We can help you complete all the necessary documents and provide services that help you quickly and easily. To begin, you must plan and rendezvous with us. You must bring the following information to your appointment:
Recent chart notes from your own doctor, with a documented diagnosis *
Card valid Colorado driver's license or ID
You have two options when making appointments. You can complete our planning form online or you can call us directly at: 719-555-1212. If you call to schedule an appointment, please leave your phone number with area code. Please speak slowly and clearly and spell your last name. One of our representatives will call you as soon as possible, usually within a few days.
After our doctor has seen and evaluated your records, he / she will determine your need for a Colorado medical marijuana card. If you qualify, he / she must sign your license and give you instructions on how to save documents in the office of the Colorado Medical Marijuana (CMMR).
* Our doctors are allowed to see patients only to determine their qualifications for the receipt of medical cannabis. They can not and will not act in any other capacity. They can act as your doctor.
We require all patients to have at least one other doctor in progress. You must be able to provide current medical records from your current doctor that documents the current state (s) for which medical marijuana may be allowed under Colorado State Law. You must obtain these documents yourself and bring them to our office. You can also authorize your medical records by fax or by mail to our office as well.
It is important not only need our doctor notes chart from 2 or 3 visits to your doctor courses that are relevant to your qualification condition (s). We do not need your complete medical record. When obtaining your medical records current please ensure they fully understand the x-ray or MRI reports (not real film), as well as your name and phone number.
